Q: What is the policy number?

Q: What is the effective date?
A: The policy will become effective on June 30, 2012 at 12:01am. Coverage will become effective on that date or the date we receive the required premium, whichever is later.

Q: What is the policy expiration date?
A: 12:00 am on September 1, 2013.

Q: If I missed the original registration or waived coverage initially and now want the coverage, how do I go about getting it?
A: Enrollment for coverage under the policy will not be accepted after October 1, 2012. Exceptions for dependent coverage will be made for the following: Adding a new spouse or dependent child within 31 days of marriage, birth, adoption or child placement with an insured for adoption. Contact the Student Services Office for enrollment into the plan.

Q: How do I know if I am eligible to take the Student Injury & Sickness Insurance?
A: Full-time students attending the Institution are eligible to participate in this plan. Part-time students are not eligible for coverage under this plan, unless they are enrolled in six credit hours or more.

Q: Can I get the insurance to cover my spouse and/or children?
A: Students may also secure family coverage. Eligible dependents are the spouse (residing with the Insured Student) and children up to age 26.

Q: What does this insurance cover?
A: This insurance covers medical expenses which arise from an accidental Injury or a Sickness which occurs while you are covered under this program.

Q: Does this policy pay all medical bills in full?
A: In most circumstances, it will not. There are exclusions and limitations in the policy, so you will need to refer to the brochure or the "Coverage" section of this website.

Q: Can I go to any doctor?
A: Yes you can. However, the Insurance Company does work with First Health, a Preferred Provider Organization network of physicians and hospitals. Going to a First Health Network Physician or Hospital would, in most cases, lower out of pocket expenses. Click on "Find a Doctor" in this web site for a directory of preferred providers.

Q: Where do I get a claim form?
A: Claim Forms are available at Student Services. You may also obtain one from clicking here.

Q: How do I file a claim?
A: Attach your itemized bills to a completed Claim Form and mail to Bollinger, Inc., P.O. Box 727, Short Hills, NJ 07078-0727. Only one Claim Form is needed per Injury or Sickness.

Q: Does this policy cover me for my annual physicals?
A: Coverage will be included for one annually physical exam with a $10 co-pay up to a maximum benefit of $125.

Q: Is there a deductible on this policy?
A: There is no policy deductible. However, there is a $15 co-pay for outpatient physician services.

Q: What if I am out of state when I get sick or injured?
A: The insurance plan will cover you for any Sickness or Injury which occurs out of state. You must follow the same procedures for filing a claim as if it had happened in New Hampshire.

Q: How do I get an I.D. card?
A: ID Cards will be mailed to the school for each student in the fall. You can also print one from this website.

Q: What is the policyâ??s maximum benefit?
A: $25,000 per Injury or Sickness

Q: What do you pay for doctorâ??s visits?
A: Outpatient visits are covered up to the Usual & Customary Charge for an Injury or Sickness. For a Sickness there is a $15 co-pay for each visit and coverage is limited to 10 visits per policy year.
