Q: How do I know if I am eligible to take the Student Injury & Sickness Insurance?
A: All students are eligible.

Q: Can I get the insurance to cover my spouse and/or children?
A: Yes. Eligible students enrolled in this insurance plan may also insure their dependents (see brochure for details). Eligible Dependents are the spouse and children under 26 years of age. Dependent Eligibility expires concurrently with that of the Insured Student.

Q: What does this insurance cover?
A: This insurance covers medical expenses which arise from an accidental Injury or a Sickness which occurs while you are covered under this program.

Q: Does this policy pay all medical bills in full?
A: There are exclusions and limitations in the policy, so you will need to refer to the brochure (See sample brochure under "Brochure").

Q: Can I go to any doctor?
A: Yes. However, if you see a physician from First Health, our nationwide network, your fees and out of pocket costs will be less.

Q: Where do I get a claim form?
A: You can print a claim form from this link or pick up a claim form at the Hampden-Sydney College Student Health Center.

Q: How do I file a claim?
A: Attach your bills to a completed Claim Form and mail to Bollinger Inc., P.O. Box 727, Short Hills, NJ 07078-0727. Only one claim form is needed per Injury or Sickness. Subsequent itemized bills may be sent in without completing another claim form. Just indicate that you are a Hampden-Sydney College student and the date of your Injury or Sickness.

Q: Does this policy cover me for my annual physicals?
A: No.

Q: Is there a deductible on this policy?
A: There is no deductible for Injury related benefits. However, there is a $25 deductible per Sickness.

Q: With this insurance must I go to the College Health Center first?
A: No, however the student is strongly encouraged to use the resources of the Health Center first where treatment will be administered, or referral issued.  A Hampden-Sydney College referral for outside care is not necessary for emergency medical services.  Please see brochure for details.

Q: What if I am out of state when I get sick or injured?
A: This insurance plan will cover you for any Sickness or Injury which occurs out of state. You must follow the same procedures in filing a claim as if it had happened in Virginia.

Q: How do I get an I.D. card?
A: Additional I.D. cards are available at the Student Health Center or one can be printed from this website.

Q: What is the policy’s maximum benefit?
A: Basic Medical Expense:
Injury Benefit: Payment will be made up to a maximum of $2,500 ($5,000 for Intercollegiate Sports on an excess basis) for each Injury incurred provided such treatment begins within 90 days after the onset of the Injury.

Sickness Benefit: Payment will be made after a $25.00 deductible (per Sickness) has been met up to $1,000 for each Sickness.

Optional Major Medical: After benefits are paid under the Basic Benefits, and after the Insured has satisfied a $100 deductible, the plan then pays 80% of all remaining eligible expenses up to a policy maximum of $10,000 (including Basic Benefits).
