Q: What is the policy number?
A: The 2012 - 2013 policy number is C517I.

 

Q: What is the effective date:
A: June 30, 2012 at 12:01am.

 

Q: What is the policy expiration date?
A: June 30, 2013 at 12:00am. Even if you leave school or graduate, your coverage will remain in force until that date.

 

Q: If I missed the original registration or waived coverage initially and now want the coverage, how do I go about getting it?
A: You can send your check with the annual premium amount of $45.00 directly to Bollinger Inc./ATT: College Dept. 200 Jefferson Park, Whippany, NJ 07981. Your coverage will begin on the date we receive your payment. If you want coverage for the second semester only, the cost is $38.00

 

Q: How do I know if I am eligible to take the Student Injury & Sickness Insurance?
A: All full time students carrying twelve or more credits are eligible to purchase this insurance.

 

Q: Can I get the insurance to cover my spouse and/or children?
A: No.

 

Q: What does this insurance cover?
A: This insurance covers medical expenses which arise from an accidental Injury or a Sickness which occurs while you are covered under this program.

 

Q: Does this policy pay all medical bills in full?
A: In most circumstances, it will not. There are exclusions and limitations in the group master policy, so you will need to refer to the brochure (See sample brochure under "Brochure").

 

Q: Can I go to any doctor?
A: Yes you can. However, the Insurance Company does work with the Consumer Health Network (CHN) which is the largest Preferred Provider Organization network of physicians and hospitals in New Jersey. Going to a CHN Network Physician or Hospital would in most cases result in lower charges which would directly lower any co-insurance or co-pays that you would have to pay. Please contact the Health Center at Essex County College for more details on the network.

 

Q: Where do I get a claim form?
A: You can pick up a claim form at the Health Center or you can print one from this link

 

Q: How do I file a claim?
A: Attach your bills to a completed Claim Form and mail to Bollinger Inc., P.O. Box 727, Short Hills, NJ 07078-0727. Only one claim form is needed per Injury or Sickness. Subsequent itemized bills may be sent in without completing another claim form. Just indicate that you are a Essex County College Student and the date of your Injury or Sickness.

 

Q: Does this policy cover me for my annual physicals?
A: For all persons age 20 and older, an annual consultation and tests are covered under the policy.

Q: Is there a deductible on this policy?
A: No, there is no deductible.

 

Q: With this insurance must I go to the College Health Center first?
A: You are encouraged to go the College Health Center first whenever that is possible. However, it is not mandatory in order to be covered by this insurance plan.

 

Q: What if I am out of state when I get sick or injured?
This insurance plan will cover you for any Sickness or Injury which occurs out of state. You must follow the same procedures in filing a claim as if it had happened here in New Jersey.

 

Q: How do I get an I.D. card?
A:  It is attached to your Student Injury & Sickness Brochure. You can cut it out and carry it with you.

 

Q: What is the policy's maximum benefit?
A: The maximum benefit is $50,000.

 

Q: What do you pay for doctors visits?
A: For Sickness the policy covers you beginning with the second visit when hospital confinement is not required or with the first visit when hospital confined. Allowance will be $50.00 for the first qualifying visit and $50.00 for each subsequent visit up to a $5,000 maximum benefit per Sickness. A visit to the school doctor will be considered the first call, when the school doctor makes a referral. For Injury the policy covers you beginning with the first visit on a Usual & Customary fee basis, not to exceed $5,000 for any one Injury.
