Q: What is the policy number?
A: The 2012-2013 policy number is C504I.

Q: What is the effective date:
A: June 30, 2012 at 12:01AM or the date Bollinger Inc. receives your premium check, whichever is later.

Q: What is the policy expiration date?
A: June 30, 2013 at 12:01AM.  Even if you leave school or graduate, your coverage will remain in force until that date.

Q: If I missed the original registration or waived coverage initially and now want the coverage, how do I go about getting it for full-time students?
A: You can send your check with the annual premium amount of $57.00 directly to Bollinger Inc. Company/ATT: College Dept. 200 Jefferson Park, Whippany, NJ 07981. Your coverage will begin on the date we receive your payment.

Q: How do I know if I am eligible to take the Student Injury & Sickness Insurance?
A: If you are a full-time student carrying twelve credit hours or part-time student carrying eight credit hours you are eligible.

Q: Can I get the insurance to cover my spouse and/or children?
A: No.

Q: What does this insurance cover?
A: This insurance covers medical expenses which arise from an accidental Injury or a Sickness which occurs while you are covered under this program.

Q: Does this policy pay all medical bills in full?
A: In most circumstances, it will not. There are exclusions and limitations in the policy, so you will need to refer to the brochure (See sample brochure under "Brochure").

Q: Can I go to any doctor?
A: Yes you can. However, the Insurance Company does work with the Consumer Health Network (CHN) which is the largest Preferred Provider Organization network of physicians and hospitals in New Jersey. Going to a CHN Network Physician or Hospital would in most cases result in lower charges which would directly lower any co-payments that you would have to pay. 

Q: Where do I get a claim form?
A: You can pick up a claim form at the Health Center or you can print one from this link

Q: How do I file a claim?
A: Attach your bills to a completed Claim Form and mail to Bollinger Inc, P.O. Box 727, Short Hills, NJ 07078-0727. Only one claim form is needed per accident or illness. Subsequent itemized bills may be sent in without completing another claim form. Just indicate that you are a Bergen Community College Student and the date of your accident or illness.

Q: Does this policy cover me for my annual physicals?
A: For all persons age 20 and older, an annual consultation and tests are covered under the policy.

Q: Is there a deductible on this policy?
A: No, there is no deductible and this insurance is primary, so you need not go to any other applicable insurance before collecting under this plan.

Q: What if I am out of state when I get sick or injured?
This insurance plan will cover you for any illness or Injury which occurs out of state. You must follow the same procedures in filing a claim as if it had happened here in New Jersey.

Q: How do I get an I.D. card?
A: The I.D. card is included in the brochure and also can be printed out on this site.

Q: What is the policy's maximum benefit?
A: The maximum benefit is $35,000 for full-time students & $2,500 for part-time students.

Q: What do you pay for doctors visits?
A: For Sickness the policy covers you beginning with the second visit when hospital confinement is not required or with the first visit when hospital confined.   Allowance will be $50 per visit, up to a  $250 maximum benefit per Sickness.  A visit to the school doctor will be considered the first call.   For accident the policy covers you beginning with the first visit on a Usual & Customary fee basis.
