Q: What is the policy number?
A: The 2012-2013 Policy number is CVT412I.
Q: What is the effective date?
A: June 30, 2012 for students selecting annual coverage. Please see the
brochure for other coverage periods. (See sample brochure under "Your
Coverage").
Q: What is the policy expiration date?
A: August 15, 2012, for students who have purchased annual coverage.
Q: If I missed the original registration or waived coverage initially and
now want the coverage, how do I go about getting it?
A: You should send your check for the applicable premium directly to
Bollinger, Inc./ATT.: College Dept, 101 JFK Parkway, Short Hills, NJ
07078. your coverage will begin on the date we receive your payment.
Q: How do I know if I am eligible to take the Student Injury & Sickness
Insurance?
A: All students will be billed for health insurance unless a waiver card is completed and returned to the Student Accounts Office.
Q: Can I get the insurance to cover my
spouse and/or children?
A: No. You may not.
Q: What does this insurance cover?
A: This insurance covers medical expenses which arise from an accidental
Injury or a Sickness which occurs while you are covered under this
program.
Q: Does this policy pay all medical bills in full?
A: In most circumstances it will not. There are deductibles, coverage
limitations and exclusions in the policy, so you will need to refer to the
brochure (See sample brochure under "Your Coverage").
Q: Can I go to any doctor?
A: Yes you may. However, in a non-emergency situation, you should go to the
Health Center first, if possible. You should seek a physician who
participates in the First Health Preferred Provider Network in order to
receive the most cost-savings.
Q: Where do I get a Claim Form?
A: You can print one from this web site (See "Claim Form").
Q: How do I file a claim?
A: Attach your bills to a completed Claim Form and mail to Bollinger,
Inc., P.O. Box 727, Short Hills, NJ 07078-0727. Only one Claim Form is
needed per Injury or Sickness.
Q: With this insurance must I go to the College's Student Health Center
first?
A: No, but you should go to the Health Center whenever possible.
Q. What if I am out of state when I get
sick or injured?
A. This insurance plan will cover you for any Sickness or Injury which
occurs out of state. You must follow the same procedures in filing a claim
as if it happened in Vermont.
Q: How do I get an I.D. Card?
A: The ID Card was
attached to the waiver card you received in the mail. One can also be
printed from this site.
Q: What is the policy's maximum benefit?
A: $3,500 per Sickness/per Injury.
