Q: What is the effective date?

A:  The policy is effective August 31, 2009.                  

 

Q:  What is the policy expiration date?

A:  The policy will expire at midnight on August 30, 2010.

 

Q:   If I missed the original registration or waived coverage initially and now want the coverage, how do I go about getting it?

A:   If you are a full time student, you are charged for the insurance on your tuition bill.

If you are a part time student, you must complete an application and submit it along

with your payment no later than October 15, 2009 for the Fall semester and February

27, 2010 for the Spring semester. If you waived coverage, you may not be eligible to purchase the coverage until the policy next renews.

 

Q: How do I know if I am eligible to take the Student Injury & Sickness Insurance?

A:  Full time and part time students are eligible to purchase the insurance.

 

Q: Can I get the insurance to cover my spouse and/or children?

A:  The policy does not provide coverage to your spouse or children.

 

Q:  What does this insurance cover?

A:  The policy provides benefits for hospital care rendered for injury or sickness.

 

Q: Does this policy pay all medical bills in full?

A:  There are limitations to the policy.  You should refer to the brochure for the details.

A:  You may print a claim form from this website.

 

Q: How do I file a claim?

A:  You may print out a claim form, complete it and mail it to the address listed on the back of the claim form. You may also complete and submit a claim on line from this website.

 

Q: Is there a deductible on this policy?

A:  No, there is no deductible on this policy.

 

Q: What if I am out of state when I get sick or injured?

A:  The policy will cover you when you are out of state.  You will follow the same procedures for filing a claim as you would if you were in state.

A:  You may print out an ID card from this website.
