Q: How do I know if I am eligible to take the Student Injury & Sickness Insurance?
A: All students are eligible.

Q: Can I get the insurance to cover my spouse and/or children?
A: Yes. Eligible students enrolled in this insurance plan may also insure their dependents. Eligible Dependents are the spouse and children up to 26 years of age. Dependent Eligibility expires concurrently with that of the Insured Student.

Q: What does this insurance cover?
A: This insurance covers medical expenses which arise from an accidental Injury or a Sickness which occurs while you are covered under this program.

Q: Does this policy pay all medical bills in full?
A: There are exclusions and limitations in the policy, so you will need to refer to the brochure (See sample brochure under "Brochure").

Q: Can I go to any doctor?
A: Yes you can. Discounts on fees for medical services are available through our Preferred Provider Network, First Health. Visit the web site for a directory of providers.

Q: Where do I get a claim form?
A: You can pick up a claim form at the Millsaps College Student Health Services Office or you can print one from this link or one will be sent to you on first notice of a claim.

Q: How do I file a claim?
A: Attach your bills to a completed Claim Form and mail to Bollinger Inc., P.O. Box 727, Short Hills, NJ 07078-0727. Only one claim form is needed per Injury or Sickness. Subsequent itemized bills may be sent in without completing another claim form. Just indicate that you are a Millsaps College student and the date of your Injury or Sickness.

Q: Does this policy cover me for my annual physicals?
A: No.

Q: Is there a deductible on this policy?
A: There is a $50 deductible for each Injury & Sickness for the Basic coverage. There is a $200 deductible for the Major Medical coverage.

Q: With this insurance must I go to the University Health Center first?
A:
No, however students are encouraged to use the Student Health Center whenever possible.

Q: What if I am out of state when I get sick or injured?
A: This insurance plan will cover you for any Sickness or Injury which occurs out of state. You must follow the same procedures in filing a claim as if it had happened in Mississippi.

Q: How do I get an I.D. card?
A: Additional I.D. cards are available at the Student Health Services Office.

Q: What is the policy’s maximum benefit?
A: Basic Medical Expense: After a $50 deductible, payment will be made up to a maximum of $1,000.00 for medical expense resulting from each Injury or Sickness occurring during the term insured. Benefits are payable at 90% in-network and 70% out-of-network.

 

Major Medical: After a $200.00 deductible has been paid, payment will be made for medical expenses incurred in excess of $1,000.00 for an Injury or Sickness, up to a combined maximum of $10,000 payable under this benefit for each Injury or Sickness. Benefits are payable at 70% in-network and 50% out-of-network.

 

Optional catastrophic may be purchased by completing the enclosed enrollment form and paying an additional premium. Optional coverage (dependent and/or catastrophic) must be purchased at the same time as the Basic & Major Medical coverage. After charges are paid under the Basic and Major Medical benefits, the plan will then pay 100% of all remaining charges up to a $50,000 per Injury or Sickness (including Basic and Major Medical benefits). If this benefit is purchased coverage for Medical Evacuation and Repatriation are included.
