Q. What is the policy #?
A. The 2008-2009 policy # is A572E.
Q. What is the effective date?
A. September 1, 2008 or the date which Bollinger
Inc. receives your premium check, whichever is later.
Q. What is the policy expiration date?
A. September 1, 2009. Even if you leave
school or graduate, your coverage will remain in force until that date.
Q. If I missed the original registration or waived coverage initially and now
want the coverage, how do I go about getting it?
A. You can send your check with
the annual premium amount of $430.00 directly to Bollinger Inc./ATT: College Dept.
101 JFK Parkway, Short Hills, NJ 07078. Your coverage will begin on the date we
receive your payment. If you want coverage for the Spring and Summer terms the amount
is $294.00.
Q. How do I know if I am eligible to take the Student Accident & Sickness Insurance?
A. All students are eligible.
Q. Can I get the insurance to cover my spouse and/or children for the full year?
A. Yes, the cost is an additional $414.00 to cover a spouse or child. Full family
coverage (student and spouse and child/children) costs a total of $860.00.
Q. What does this insurance cover?
A. This insurance covers medical expenses
which arise from an accidental Injury or a Sickness which occurs while you are covered
under this program.
Q. Does this policy pay all medical bills in full?
A. In most circumstances,
it will not. There are exclusions and limitations in the policy, so you will need
to refer to the brochure
Q. Can I go to any doctor?
A. Yes you can. Discounts on fees for medical services
are available through our Preferred Provider Network,

Q. Where do I get a claim form?
A. You can pick up a claim form at the College
Heath Center, or or one will be mailed to you on first notice of claim, or you can
print one from this web site.
Q. How do I file a claim?
A. Attach your bills to a completed Claim Form and
mail to Bollinger Inc., P.O. Box 727, Short Hills, NJ 07078. Only one claim form
is needed per accident or illness. Subsequent itemized bills may be sent in without
completing another claim form. Just indicate that you are a Washington and Jefferson
College Student and the date of your accident or illness.
Q. Does this policy cover me for my annual physicals?
A. Yes, for Women's
Preventive Health Services Benefit
Q. Is there a deductible on this policy?
A. Yes, the deductible is $50.00
which is waived when initial treatment is begun at the Student Health Center.
Q. With this insurance must I go to the Student Health Center first?
A. You
are encouraged to go to the Student Health Center first whenever that is possible.
However, it is not mandatory in order to be covered by this insurance plan.
Q. What if I am out of state when I get sick or injured?
A. This insurance
plan will cover you for any illness or Injury which occurs out of state. You must
follow the same procedures in filing a claim as if it had happened here in Pennsylvania.

A. It is attached to your Student Accident &
Sickness Brochure or click here.
Q. What is the policy's maximum benefit?
A. The maximum benefit is $25,000.
Q. What do you pay for doctors visits?
A. For Sickness, the policy covers
you beginning with the first visit when hospital confinement is not required and
with the first visit when hospital confined. Allowance will be the usual and customary
charge for the first qualifying visit and usual and customary charge for each subsequent
visit up to $2,000.00 maximum benefit per Sickness. For an accident the policy covers
you upon the first visit on a usual & customary basis, up to a $2,000 maximum per
Injury.
