A: Yes, but only if you purchase the insurance for yourself. You can print an

go to any doctor?
A: Yes you can. However the Insurance Company has a relationship with the
Consumer Health Network (CHN)
which is the largest Preferred Provider Organization network of
physicians and hospitals in New Jersey. Going to a
CHN Network Physician or Hospital would in
most cases result in lower charges which would directly lower any co-payments
that you would have to pay. Please visit

A: You can pick up a claim form at Student Health Services or you can

Q: How do I file a claim?
A: Attach your bills to a completed Claim Form and mail to:

Only one claim form is needed per
Injury
or Sickness. Subsequent itemized bills may be sent in without completing another
claim form. Just indicate that you are a College of New Jersey Student and the date
of your Injury or Sickness. You may also complete and submit a claim form

Q: Does this policy cover me for my annual physicals?
A: For all persons age 20 and older, an annual consultation and tests are
covered under the Policy up to the specified limits.

A: No, there is no deductible.

go to the University Health Center first?
A. You are encouraged to go the the College Health Center first whenever that is
possible. However, it is not mandatory in order to be covered by this insurance
plan.

A: This insurance plan will cover you for any Sickness or Injury which occurs out
of state. You must follow the same procedures in filing a claim as if it had
happened here in New Jersey.
