Q. If I missed the original registration or waived coverage initially and now want the coverage, how do I go about getting it?
A. You can send your check with the semester premium amount of $120.00 directly to:

Q: How do I know if I am eligible to take the Student Accident & Sickness Insurance?
A: If you are a full-time or part-time student you are eligible.

Q: Can I get the insurance to cover my spouse and/or children?
A: Yes, but only if you purchase the insurance for yourself. You can print an application from this web site.

Q: Can I go to any doctor?
A: Yes you can. However the Insurance Company has a relationship with the
Consumer Health Network (CHN) which is the largest Preferred Provider Organization network of physicians and hospitals in New Jersey. Going to a CHN Network Physician or Hospital would in most cases result in lower charges which would directly lower any co-payments that you would have to pay. Please visit www.chn.com for more details on the network.

Q: Where do I get a claim form?
A: You can pick up a claim form at the Health Center or you can
print one from the following page.

Q: How do I file a claim?
A: Attach your bills to a completed Claim Form and mail to:

Only one claim form is needed per accident or illness. Subsequent itemized bills may be sent in without completing another claim form. Just indicate that you are a College of New Jersey Student and the date of your accident or illness.

Q: Does this policy cover me for my annual physicals?
A: For all persons age 16 and older, an annual consultation and tests are covered under the Policy up to the specified limits.

Q: Is there a deductible on this policy?
A: No, there is no deductible.

Q. With this insurance must I go to the University Health Center first?
A. You are encouraged to go the the College Health Center first whenever that is possible. However, it is not mandatory in order to be covered by this insurance plan.

Q: What if I am out of state when I get sick or injured?
A: This insurance plan will cover you for any illness or Injury which occurs out of state. You must follow the same procedures in filing a claim as if it had happened here in New Jersey.

Q: How do I get an I.D. card?
A: If you are covered under the insurance plan and do not already have an insurance I.D. card, you can print one from this website.

Q: What is the policy's maximum benefit?
A: The maximum benefit is $50,000.00

Q: What do you pay for doctors visits?
A: Please refer to the coverage section of this site or your printed brochure to address the specific physician limits.

Q: Is there a prescription charge?
A. The policy will pay for a maximum of $300.00 of prescription drugs per policy year.

Q: How can I waive coverage if I have other insurance?
A: You must complete a waiver form and provide proof of other insurance prior to the deadline required by the college.
