Q. What is the policy #? A. The 2008-2009
policy # is CTX130E.
Q. What is the effective date? A. August
13, 2008 for students selecting annual coverage.
Q. If I missed the original registration
or waived coverage initially and now want the coverage, how do I go about
getting it? A. You should send your check for the applicable premium
directly to Bollinger, Inc./ATT.: College Dept., 101 JFK Parkway, Short
Hills, NJ 07078. Your coverage will begin on the date we receive your payment.
Q. How do I know if I am eligible to take
the Student Injury & Sickness Insurance? A. If you are a full-time student
taking 6 or more credits at LeTourneau University you are automatically
enrolled in this plan unless you provide proof of alternate coverage.
Q. Can I get the insurance to cover my spouse
and/or children? A. Yes, please refer to the brochure for details.
Q. What does this insurance cover? A.
This insurance covers medical expenses which arise from an accidental Injury
or a Sickness which occurs while you are covered under this program.
Q. Does this policy pay all medical bills
in full? A. In most circumstances, it will not. There are exclusions
and limitations in the policy, so you will need to refer to the brochure.
Q. May I go to any doctor? A. Yes, you
may. However, in a non-emergency situation you are encouraged to go to the
Health Center first whenever possible, and then to a physician participating
in the First Health Preferred Provider Network (See listing of preferred
providers under "First Health").
Q. Where do I get a claim form? A. You
can print one out from this web site (See "Claim Form").
Q. How do I file a claim? A. Attach your
itemized bills to a completed Claim Form and mail to Bollinger, Inc., P.O.
Box 727, Short Hills, NJ 07078-0727. Only one claim form is needed per Injury
or Sickness.
Q. What if I am out of state when I get sick
or injured? A. This insurance plan will cover you for any illness or
Injury which occurs out of state. You must follow the same procedures in
filing a claim as if it had happened here in Texas.
Q. How do I obtain an I.D. card? A. It
is sent with your Student Accident & Sickness Brochure. You can fill it
out and carry it with you.
Q. What is the policy’s maximum benefit?
A. The maximum benefit is $15,000.
