Q. What is the policy #?
A. The 2008-2009 policy # is CMA804E.

Q. What is the effective date?
A. August 1, 2008 for students selecting annual coverage. Please see the brochure for other coverage periods (See sample brochure under "Brochure").

Q. If I missed the original registration or waived coverage initially and now want the coverage, how do I go about getting it?
A. You should send your check for the applicable premium directly to Bollinger, Inc./ATT.: College Dept., 101 JFK Parkway, Short Hills, NJ 07078. Your coverage will begin on the date we receive your payment.

Q. How do I know if I am eligible to take the Student Injury & Sickness Insurance?
A. If you are a full-time or qualifying part-time student at College of the Holy Cross, Massachusetts state law requires you to either purchase this insurance plan or provide proof that you are enrolled in comparable coverage for the full academic year.

Q. Can I get the insurance to cover my spouse and/or children?
A. No.

Q. What does this insurance cover?
A. This insurance covers medical expenses which arise from an accidental Injury or a Sickness which occurs while you are covered under this program.

Q. Does this policy pay all medical bills in full?
A. In most circumstances, it will not. There are exclusions and limitations in the policy, so you will need to refer to the brochure (See sample brochure under "Brochure").

Q. May I go to any doctor?
A. Yes, you may. However, in a non-emergency situation you are encouraged to go to the Health Center first whenever possible, and then to a physician participating in the First Health Preferred Provider Network.

Q. Where do I get a claim form?
A. You can print one out from this web site (See "Claim Form").

Q. How do I file a claim?
A. Attach your itemized bills to a completed Claim Form and mail to Bollinger, Inc., P.O. Box 727, Short Hills, NJ 07078-0727. Only one claim form is needed per accident or illness.

Q. What if I am out of state when I get sick or injured?
A. This insurance plan will cover you for any illness or Injury which occurs out of state. You must follow the same procedures in filing a claim as if it had happened here in Massachusetts.

Q. How do I obtain an I.D. card?
A. It is attached to your Student Injury & Sickness Brochure (see the inside of the cover page). You can cut it out and carry it with you.

Q. What is the policy’s maximum benefit?
A. The maximum benefit is $50,000.

A. You can call Bollinger, Inc. at 1-866-267-0092 and ask to be connected to the College Claims Department and speak to Jane Farrell.
