Q: How do I know if I am eligible to take the Student Accident & Sickness
Insurance?
A: All registered students are automatically enrolled at registration, unless
proof of comparable coverage is given. Waivers must be submitted to EMU
prior to September 20, 2009.
Q: Can I get the insurance to cover my spouse and/or children?
A: Yes. Eligible students who enroll may also insure their Dependents. Eligible
Dependents are the spouse and unmarried children under 19 years of age who are
not self supporting. Dependents eligibility expires concurrently with that of
the Insured Student. For Dependent coverage and enrollment information, please
call Bollinger, Inc. toll free at 1-866-267-0092, or print an Enrollment
Application from the Student Insurance web site, www.BollingerInsurance.com/emu.
Q: Is there a deductible on this policy?
A: There is a $100 accident & $300 sickness deductible.
Q: With this insurance must I go to the University Health Center first?
A: No, but EMU asks that students report to Student Health Services first unless away
from Campus or an emergency situation.
Q: How do I get an I.D. card?
A: Detach the I.D. card from the Enrollment Application and keep for your
records. Extra enrollment Applications are available on this site.
Q: What does this insurance cover?
A: This insurance covers medical expenses which arise from an accidental Injury
or Sickness which occurs while you are covered under the program.
Q: Does this policy pay all medical bills in full?
A: No, there are exclusions and limitations in the policy, so you will need to
refer to the brochure.
Q: Can I go to any doctor?
A: Yes you can. However, the Bollinger, Inc. does work with First Health, a Preferred
Provider Organization network of physicians and hospitals. Going to a First Health
Network Physician or Hospital would in most cases result in lower charges which
would directly lower any co-payments that you would have to pay.

A: Print one from this website or call Bollinger, Inc. at 1-866-267-0092.
Claim forms will also be available at Eastern Mennonite University.
Q: How do I file a claim?
A: Attach your bills to a completed Claim Form and mail to Bollinger Inc., P.O.
Box 727, Short Hills, NJ 07078-0727. One Claim Form is needed per accident or
illness. Subsequent itemized bills may be sent in without completing another
Claim form, if they are related to the original claim.
