Q. What is the policy #?
A. The 2012-2013 policy # is A576I

Q. What is the effective date?
A. June 30, 2012 at 12:01am or the date which Bollinger Inc. receives your premium check, whichever is later.

Q. What is the policy expiration date?
A. August 1, 2013 at 12:00am. Even if you leave school or graduate, your coverage will remain in force until that date.

Q. If I missed the original registration or waived coverage initially and now want the coverage, how do I go about getting it?
A. You can send your check with the annual premium amount of $294. directly to Bollinger Inc. Att: College Dept. 200 Jefferson Park, Whippany, NJ 07981. Your coverage will begin on the date we receive your payment.

Q. How do I know if I am eligible to take the Student Injury & Sickness Insurance?
A. If you are enrolled in at least twelve (12) credit hours of classes and actively attending classes for at least 31 days after the date for which coverage is purchased you will be insured for the period for which premium has been paid, including interim vacations.

Q. Can I get the insurance to cover my spouse and/or children?
A. No

Q. What does this insurance cover?
A. This insurance covers medical expenses which arise from an accidental Injury or a Sickness which occurs while you are covered under this program.

Q. Does the Policy pay all medical bills in full?
A. In most circumstances, it will not. There are exclusions and limitations in the policy, so you will need to refer to the brochure.

Q. Can I go to any doctor?
A. Yes you can.

Q. Where do I get a claim form?
A. You can pick up a claim form at Student Health or you can print one out from this web site

Q. How do I file a claim?
A. Attach your bills to a completed Claim Form and mail to Bollinger Inc., P.O. Box 727, Short Hills, NJ 07078-0727. Only one claim form is needed per Injury or Sickness. Subsequent itemized bills may be sent in without completing another claim form. Just indicate that you are a Chestnut Hill College Student and the date of your Injury or Sickness.

Q. Does the Policy cover me for my annual physicals?
A. No, except for Women’s Preventive Health Services Benefit.

Q. Is there a deductible on the Policy?
A. No, there is no deductible and this insurance is primary so you need not go to any other applicable insurance before collecting under this plan.

Q. With this insurance must I go to the College Health Center first?
A. You are encouraged to go to Student Health first whenever that is possible . However, it is not mandatory in order to be covered by this insurance plan.

Q. What if I am out of state when I get sick or injured?
A. This insurance plan will cover you for any Sickness or Injury which occurs out of state. You must follow the same procedures in filing a claim as if it had happened here in Pennsylvania.

Q. What is the policy's maximum benefit?
A. The maximum benefit is $25,000 for each Injury & $50,000 for each Sickness.

Q. What do you pay for doctors visits?
A. For Sickness, the policy covers you for $75 per visit to $750 maximum benefit per Sickness. For an Injury the policy covers you upon the first visit on a Usual & Customary basis not to exceed $2,500 for any one Injury.
