Q: What is the policy number?
A: The 2012-2013 policy number is C502I.

Q: What is the effective date:
A: August 30, 2012 at 12:01am or the date Bollinger Inc. receives your premium check, whichever is later.

Q: What is the policy expiration date?
A: August 30, 2013 at 12:00am. Even if you leave school or graduate, your coverage will remain in force until that date.

Q: How do I know if I am eligible to take the Student Injury & Sickness Insurance?
A: All full time and part time students are eligible and enrollment is conducted by the college.

What constitutes full-time?
Full time student enrollment is handled through the college.

What constitutes part-time?
Part time students should enroll directly through Bollinger.

Q: Can I get the insurance to cover my spouse and/or children?
A: Yes, only dependents of full-time students are eligible for insurance.

Q: What does this insurance cover?
A: This insurance covers medical expenses which arise from an accidental Injury or a Sickness which occurs while you are covered under this program.

Q: Does this policy pay all medical bills in full?
A: In most circumstances, it will not. There are exclusions and limitations in the policy, so you will need to review and select your brochure .

Q: Can I go to any doctor?
A: Yes you can. However, the Insurance Company does work with the Consumer Health Network (CHN) which is the largest Preferred Provider Organization network of physicians and hospitals in New Jersey. Going to a CHN Network Physician or Hospital would in most cases result in lower charges which would directly lower any co-payments that you would have to pay. Please contact the Health Center at the College for more details on the network.

Q: Where do I get a claim form?
A: You can pick up a claim form at the Health Center or you can print one from this link.

Q: How do I file a claim?
A: Attach your bills to a completed Claim Form and mail to Bollinger Inc., P.O. Box 727, Short Hills, NJ 07078-0727. Only one claim form is needed per Injury or Sickness. Subsequent itemized bills may be sent in without completing another claim form. Just indicate that you are a County College of Morris Student and the date of your Injury or Sickness.

Q: Does this policy cover me for my annual physicals?
A: For all persons age 20 and older, an annual consultation and tests are covered under the policy.

Q: Is there a deductible on this policy?
A: No, there is no deductible and this insurance is primary, so you need not go to any other applicable insurance before collecting under this plan.

Q: With this insurance must I go to the College Health Center first?
A: You are encouraged to go the University Health Center first whenever that is possible. However, it is not mandatory in order to be covered by this insurance plan.

Q: What if I am out of state when I get sick or injured?
This insurance plan will cover you for any Sickness or Injury which occurs out of state. You must follow the same procedures in filing a claim as if it had happened here in New Jersey.

Q: How do I get an I.D. card?
A: I.D. cards are attached to the Student Insurance Program brochure you receive at the beginning of the school year.

Q: What is the policy's maximum benefit?
A: The maximum benefit is $50,000.

Q: What do you pay for doctors visits?
A: The allowance will be $50 for the first qualifying visit and $25 for each subsequent visit, up to a $250 maximum for any one Sickness.
