the effective date?
A:
The Master Policy is effective on
August 1, 2010. Coverage will become effective on that date or the date we
receive the required premium, whichever is later.
For students covered for the Second Semester Only, their coverage will be
effective January 1, 2010.
Q: What is
the policy expiration date?
A: 12:01 a.m.
August 1, 2011.
Q: How do I
know if I am eligible to take the Student Accident & Sickness Insurance?
A:
St.
Anselm mandates that all full-time students must enroll in the Student Health
Insurance Plan or certify that they have comparable coverage in another plan.
Coverage is available for part-time students on an optional basis.
All international students are automatically enrolled in the plan and may
not waive this insurance.
Q: Can I
get the insurance to cover my spouse and/or children?
A:
Yes. Coverage is available
for the student’s spouse and children.
A Schedule of Premium Rates is included in the brochure.
Q: What
does this insurance cover?
A:
This insurance covers medical
expenses which arise from an accidental Injury or a Sickness which occurs while
you are covered under this program.
Q: Does
this policy pay all medical bills in full?
A:
In most circumstances, it will
not. There are exclusions and limitations in the policy, so you will need to
refer to the brochure or the "Coverage" section of this website.
Q: Can I go
to any doctor?
A:
Yes you can. However, the
Insurance Company does work with First Health, a Preferred Provider Organization
network of physicians and hospitals. Going to a First Health Network Physician
or Hospital would, in most cases, lower out of pocket expenses. Click on "Find a
Doctor" in this web site for a directory of preferred providers.

I get a claim form?

Claim Forms are available at Health Services. You may also obtain one
from
clicking here.
Q: How do I
file a claim?
A:
Send
a completed Claim Form to Bollinger, Inc.,
P.O. Box 727,
Short Hills,
NJ
07078-0727.
Only one
Claim Form is needed per accident or
illness.
Q: Does
this policy cover me for my annual physicals?
A:
No it does not. Unless stated
otherwise in the brochure, this policy only pays for expenses resulting from a
covered injury or illness.
Q: Is there
a deductible on this policy?
A:
Under Basic Accident Benefits, there is a $150 policy year deductible.
Q: With
this insurance must I go to the
College
Health
Center
first?
A:
This is not a requirement. However, we do recommend taking advantage of the services provided by the
Health
Center
whenever possible.
Q: What if
I am out of state when I get sick or injured?
A:
This insurance plan will cover
you for any illness or injury which occurs out of state. You must follow the
same procedures in filing a claim as if it had happened here in MA.
Q: How do I
get an I.D. card?
A:
ID Cards will be mailed to the
school for each student in the fall.
Q: What is
the policy’s maximum benefit?
A:
$65,000
Q: What do
you pay for doctor’s visits?
A:
When your sickness requires the services of a doctor, other than a
surgeon, while not confined to a hospital, we will pay the Usual and Customary
Charge up to a maximum of $100 per visit, limited to one visit per day.
