Q: How do I
know if I am eligible to take the Student Accident & Sickness
Insurance?
A: All domestic students attending Alamo Community College District
who are
enrolled in Plan 1, are eligible to enroll in this Sickness and
Injury insurance plan. Please see “Plan 2 Enrollment Card”
to enroll on-line.
Q: Can I
get the insurance to cover my spouse and/or children?
A: Yes, you may. Please see “Plan 2 Enrollment Card” to
enroll on-line.
Q: What
does this insurance cover?
A: This insurance covers medical expenses which arise from an
accidental Injury or Sickness which occurs while you are covered
under this insurance program.
Q: Does
this policy pay all medical bills in full?
A: In most circumstances, it will not. There are exclusions and
limitations in the policy, so you will need to refer to the brochure
(See sample brochure under “Plan 2 Brochure”).
Q: Can I go
to any doctor?
A: Yes, you may. However, you are encouraged to go to a
physician participating in the
First Health
Preferred
Provider Network (See listing of preferred providers under “First Health”).
Q: Where do
I get a claim form?
A: You can print one from this web
site (see "Claim
Form") or one will be
sent to you upon our receipt of notice of your claim.
Q: How do I
file a claim?
A: Attach your itemized bills to a completed
Claim Form
and mail to Bollinger, Inc., P.O. Box 727, Short Hills, NJ
07078-0727. Only one
Claim Form
is needed per accident or illness.
Q: With
this insurance must I go to the College Health Center first?
A: No, you need not. However, you are encouraged to go to
a physician participating in the
First Health
Preferred Provider Network (See listing of preferred providers under
“First Health”).
Q: What if
I am out of state when I get sick or injured?
A: This insurance plan will cover you for any illness or Injury
which occurs out of state. You must follow the same procedures in
filing a claim as if it had happened here in Texas.
Q: How do I
get an I.D. card?
A: It is attached to the Brochure. You can cut it out and carry it
with you. You can also print
one from this web site.
Q: What is
the policy's maximum benefit?
A: $25,000.
