Q: What is the policy number?
A: The 2012-2013 policy number is C510I
Q: What is the effective date:
A: June 30, 2012 at 12:01am.or the date Bollinger, Inc. receives your premium check,
whichever is later.
Q: What is the policy expiration date?
A: June 30, 2013 at 12:00am. Even if you leave school or graduate, your coverage will remain in
force until that date.
Q: If I missed the original registration or
waived coverage initially and now want the coverage, how do I go about getting it?
A: You can send your check with the annual premium amount of $85.00 directly to Bollinger,
Inc. /Att: College Dept. 101 JFK Parkway, Short Hills, NJ, 07078. Your coverage will
begin on the date we receive your payment. If you want coverage for the second semester
only, the cost is $61.00
Q: How do I know if I am eligible to take the
Student Injury & Sickness Insurance?
A: If you are a registered full-time student you are
eligible.
Q: Can I get the insurance to cover my spouse
and/or children?
A: No. Dependent coverage is not available.
Q: What does this insurance cover?
A: This insurance covers medical expenses which arise from an accidental Injury or a
Sickness which occurs while you are covered under this program.
Q: Does the Policy pay all medical bills in
full?
A: In most circumstances, it will not. There are exclusions and limitations in
the policy, so you will need to refer to the brochure.
Q: Can I go to any doctor?
A: Yes you can. However, the Insurance Company does work with the Consumer Health Network
(CHN) which is the largest Preferred Provider Organization network of physicians and
hospitals in New Jersey. Going to a CHN Network Physician or Hospital would in most cases
result in lower charges which would directly lower any co-payments that you would have to
pay. Please contact Bollinger, Inc. for more details on the network.

Q: Where do I get a claim form?
A: You can print one from this link.
Q: How do I file a claim?
A: Attach your bills to a completed
Claim
Form and mail to Bollinger, Inc. P.O. Box 727,
Short Hills, NJ 07078-0727. Only one
claim
form is needed per Injury or Sickness.
Subsequent itemized bills may be sent in without completing another
claim form. Just
indicate that you are a Salem Community College Student and the date of your
Injury or
Sickness.
Q: Does the Policy cover me for my annual
physicals?
A: For all persons age 20 and older, an annual consultation and tests are covered under
the policy.
Q: Is there a deductible on the Policy?
A: No, there is no deductible and this insurance is primary so you need not go to any
other applicable insurance before collecting under this plan.
Q: With this insurance must I go to the
College Health Center first?
A: No, it is not mandatory in order to be covered by this insurance plan.
Q: What if I am out of state when I get sick
or injured?
This insurance plan will cover you for any Sickness or Injury which occurs out of state.
You must follow the same procedures in filing a claim as if it had happened here in New
Jersey.
Q: How do I get an I.D. card?
A: It is attached to your Student Injury and Sickness brochure. You may
also print one from this website.
Q: What is the policy's maximum benefit?
A: The maximum benefit is $15,000.00
Q: What do you pay for doctors visits?
A: For Sickness the policy covers you beginning with the second visit when hospital
confinement is not required or with the first visit when hospital confined. Allowance will
be $250.00 maximum benefit per Sickness. A visit to the school doctor will be considered
the first call, (when the school doctor makes a referral). For Injury the policy covers
you beginning with the first visit on a Usual & Customary fee basis.
