A. For Domestic Students the policy # is C535H.
A. For International Students the policy # is C935H.

Q. What is the effective date?
A. Coverage is effective as of 8/15/11 at 12:01am.

Q. What is the policy expiration date?
A. Coverage expires as of 8/15/12 at 12:00am.

Q. How do I know if I am eligible to take the Student Injury & Sickness Insurance?
A. All FDU students are eligible.

Q. Can I get the insurance to cover my spouse and/or children?
A. Yes, but only if you are covered.

Q. What does this insurance cover?
A. Accidental injuries and illness.

Q. Can I go to any doctor?
A. Yes, but you can best utilize the Plan by visiting a CHN provider.

Q. Where do I get a claim form?
A. You can pick up a claim form at the Health Center or you can print one from this web site.

Q. How do I file a claim?
A. Attach your bills to a completed Claim Form and mail to Bollinger Inc., P.O. Box 727, Short Hills, NJ 07078-0727. Only one claim form is needed per Injury or Sickness. Subsequent itemized bills may be sent in without completing another claim form. Just indicate that you are a FDU Student and the date of your Injury or Sickness.

Q. Does the Policy cover me for my annual physicals?
A. Yes, within the limits shown in the Certificate of Coverage.

Q. Is there a deductible on the Policy?
A. No

Q. With this insurance should I go to the College Health Center first?
A. In Non Emergency situations, you should utilize the College Health Center.

Q. What if I am out of state when I get sick or injured?
A. Seek medical treatment, and submit the charges. The policy will cover you as if the Injury or Sickness occurred in New Jersey.

Q. What is the policy's maximum benefit?
A. $100,000 per Injury or Sickness.
