Q: How do I know if I am eligible to take the Student Accident & Sickness Insurance?
A: All registered undergraduate students taking 12 or more credit hours , all international students, all students participating in intercollegiate sports and all athletic team graduates assistants are automatically enrolled in the plan unless they waive coverage by the waiver deadline. All other students may voluntarily purchase the plan during the open enrollment period.

Q: Can I get the insurance to cover my spouse and/or children?
A: Yes, coverage for dependents is available. It is available at www.BollingerColleges.com/Cumberlands and it must be paid for via credit card on line.

Q: What does this insurance cover?
A: Sickness and Injury are covered by this plan subject to the plan’s terms, conditions, and benefits.

Q: Does this policy pay all medical bills in full?
A: No, in most cases medical bills are not paid in full and there is an out of pocket expense.

Q: Can I go to any doctor?
A: Yes, you may go to any doctor. However, discounts on fees for services rendered are available by using First Health Providers. First Health is a nationwide preferred provider organization. You can find a First Health provider at www.firsthealth.com

Q: How do I file a claim?
A: Claims can be submitted in one of two ways. 1. Electronically at www.BollingerColleges.com/Cumberlands or 2. A claim form can be downloaded and printed at www.Bollingercolleges.com/Cumberlands which should then be completed and sent to Bollinger. Claims should be submitted to Bollinger ASAP so that bollinger can log them into their software system to help assure prompt payment. Bills and EOB’s can be sent at a later date

Q: Does this policy cover me for my annual physicals?
A: Annual physicals are not covered

Q: What if I am out of state when I get sick or injured?
A: Seek medical assistance.

Q: What is the policy’s Benefit Maximum?
A: the Benefit Maximum is $ 25,000 per sickness or injury

Q: What do you pay for doctor’s visits?
A: Inpatient Physician Visits are paid at 100% of the PPO Allowance after a $ 50 per visit in network and they are paid at 80% of the Usual and Customary allowance after a $50 copay out of network.
