Q: What is the effective date:
A: August 10, 2011 at
12:01 a.m.
Q: What is the policy expiration date?
A: August 10, 2012 at 12:00 a.m.. Even if you leave school or graduate, your coverage will remain in
force until that date.
Q: If I missed the original registration or
waived coverage initially and now want the coverage, how do I go about getting it?

Q: How do I know if I am eligible to take the
Student Accident & Sickness Insurance?
A: If you are a full time student taking 12 or more credit hours, part-time student
taking 6 or more credit hours, or graduate student, you are
eligible.
Q: Can I get the insurance to cover my spouse
and/or children?
A: Yes, the cost to cover your spouse is $1,707.00 annually. The cost to cover each child is
$1,233.00 annually.
Q: What does this insurance cover?
A: This insurance covers medical expenses which arise from an accidental Injury or a
Sickness which occurs while you are covered under this program.
Q: Does this policy pay all medical bills in
full?
A: In most circumstances, it will not. There are exclusions and limitations in the policy,
so you will need to refer to the brochure.
Q: Can I go to any doctor?
A: Yes you can. However, the Insurance Company does work with
First Health, a Preferred Provider
Organization network of physicians and hospitals. Going to a
First Health Network Physician or
Hospital would in most cases result in lower charges which would directly lower any
co-payments that you would have to pay. Please contact the Health Center at Defiance
College for more details on the network, or visit the
First Health
Directory on the web to find a provider.
Q: Where do I get a claim form?
A: You can pick up a claim form at the Health Center or you can
print one from this link.
Q: How do I file a claim?
A: Attach your bills to a completed
Claim
Form and mail to Bollinger, Inc., P.O. Box
727, Short Hills, NJ 07078-0727. Only one
claim form is needed per Injury
or Sickness.
Subsequent itemized bills may be sent in without completing another claim form. Just
indicate that you are a Defiance College Student and the date of your Injury or
Sickness.
Q: Does this policy cover me for my annual
physicals?
A: No.
Q: Is there a deductible on this policy?
A: No, there is no deductible and this insurance is primary, so you need not go to any
other applicable insurance before collecting under this plan.
Q: With this insurance must I go to the
College Health Center first?
A: You are encouraged to go the University Health Center first whenever that is
possible. No payment will be made for services normally provided without charge by the
Health Center.
Q: What if I am out of state when I get sick
or injured?
This insurance plan will cover you for any Sickness or Injury which occurs out of state.
You must follow the same procedures in filing a claim as if it had happened here in Ohio.
Q: How do I get an I.D. card?
A: ID cards are incorporated into the brochure
and this web site. Please click here to print
one for your records.
Q: What is the policy's maximum benefit?
A: The maximum benefit is $50,000.00
Q: What do you pay for doctors visits?
A: Inpatient- physician expenses are limited to one visit per day. The
maximum we will pay is $35.00 per visit up to $350.00 in total. Outpatient-
physician expenses are paid under Miscellaneous Expenses.
