Q. What is the policy #?
A. The 2011-2012 policy # is
A569H.
Q. What is the effective date?
A. August 22, 2011 or the date which Bollinger Inc.
receives your premium check,
whichever is later.
Q. What is the policy expiration date?
A. August 22, 2012. Even if you leave school or graduate, your coverage will remain in
force until that date.
Q. If I missed the original registration or waived coverage
initially and now want the coverage, how do I go about getting it?
A. You can send your check with the annual premium amount of $475.00 directly to Bollinger Inc. Company/ATT: College Dept. 101 JFK Parkway, Short Hills, NJ 07078. Your
coverage will begin on the date we receive your payment. If you want coverage for the
second semester only the cost is $316.00.
Q. Can I get the insurance to cover my spouse and/or
children?
A. No.
Q. What does this insurance cover?
A. This insurance covers medical expenses which arise from an accidental Injury or a
Sickness which occurs while you are covered under this program.
Q. Does the Policy pay all medical bills in full?
A. In most circumstances, it will not. There are exclusions and limitations in the policy,
so you will need to refer to the brochure (See sample brochure under
"Brochure").
Q. Can I go to any doctor?
A.
Yes you can.
However, if you see a physician from First Health, our nationwide network, your
fees and out of pocket costs will be less. You can obtain a complete listing of

Q. Where do I get a claim form?
A. You can pick up a claim form at the University Health Center or you can
print one from this web site.
Q. How do I file a claim?
A. You may file a claim on this
website.
Q. Does the Policy cover me for my annual physicals?
A. Yes, $200 allowance per year for an annual wellness exam.
Q. Is there a deductible on the Policy?
A. Yes, $50 per injury and $150 Deductible per policy year for sickness
benefits.
Q. With this insurance must I go to the University Health
Center first?
A. You are encouraged to go to the University Health Center first whenever that
is possible. However, it is not mandatory in order to be covered by this insurance plan.
Q. How do I get an I.D. card?
A. Once signed up and paid for, you can obtain your ID Card from Health
Services.
Q. What is the policy's maximum benefit?
A. The maximum benefit is $25,000.
Q. What do you pay for doctors visits?
A. For Sickness, the policy covers $50.00 for the first qualifying visit and $50.00 for each subsequent visit up to $1,000
maximum benefit per Sickness. For an accident the policy covers you upon the first visit
on a usual & customary basis, not to exceed $1,000 for any one Injury.

Q. How do I know if I am eligible to take the Student
Accident & Sickness Insurance?
A. Plan is mandatory for all International Students
