Q. What is the policy #?
A. The 2011-2012 policy # is CMI304H.Q. What is the effective
date?
A. August 20, 2011 for students selecting annual
coverage. Please see the brochure for other coverage periods
(See sample brochure under "Brochure").
Q. If I missed the original
registration or waived coverage initially and now want the
coverage, how do I go about getting it?
A. You should
send your check for the applicable premium directly to Bollinger,
Inc./ATT.: College Dept., 101 JFK Parkway, Short Hills,
NJ 07078. Your coverage will begin on the date we receive
your payment.
Q. How do I know if I am
eligible to take the Student Injury & Sickness Insurance?
A. If you are a full-time or qualifying part-time student
carrying 6 or more credits at Andrews University you are
required to either purchase this insurance plan or provide
proof that you are enrolled in comparable coverage for the
full academic year.
Q. Can I get the insurance
to cover my spouse and/or children?
A. Yes.
Q. What does this insurance
cover?
A. This insurance covers medical expenses which
arise from an accidental Injury or a Sickness which occurs
while you are covered under this program.
Q. Does this policy pay all
medical bills in full?
A. In most circumstances, it will
not. There are exclusions and limitations in the policy,
so you will need to refer to the brochure.
Q. May I go to any doctor?
A. Yes, you may. However, in a non-emergency situation you
are encouraged to go to the Health Center first whenever
possible, and then to University Medical Specialists, or
a physician participating in the First Health Preferred
Provider Network (See listing of preferred providers under
"First Health").
Q. How do I file a claim?
A. Send all itemized bills to Bollinger, Inc., P.O. Box
727, Short Hills, NJ 07078-0727. A claim can also be
filed on this website.
Q. What if I am out of state
when I get sick or injured?
A. This insurance plan will
cover you for any illness or Injury which occurs out of
state. You must follow the same procedures in filing a claim
as if it had happened here in Michigan.
Q. How do I obtain an I.D.
card?
A. An ID Card will be sent to you by Collegiate
Risk Management shortly after the University sends a plan
enrollment list to Bollinger, Inc.
Q. What is the policy’s maximum
benefit?
A. The maximum benefit is $25,000. An
Optional Major Medical benefit is available
through Collegiate Risk Management. This optional
major medical benefit is not associated with Bollinger,
Inc. or Monumental Life Insurance Company in any way.
