Q. How do I know if I am eligible to take the student Accident & Sickness Insurance?
A. All students enrolled at Lynn University are automatically enrolled in the Mandatory Accident Benefits. All students currently enrolled at Lynn University may purchase the Optional Basic and Major Medical Benefits by completing the enrollment card and returning it to Bollinger with payment.

Q. Can I get the insurance to cover my spouse and/or children?
A. No, this plan is for students only.

Q. What does this insurance cover?
A. This insurance covers medical expenses which arise from an accidental Injury or a Sickness which occurs while you are covered under this program.

Q. Does this policy pay equal medical bills in full?
A. In most circumstances, it will not. There are exclusions and limitations in the policy, so you'll need to refer to the brochure.

Q. Can I go to any doctor?
A. Yes, you may. However you are encouraged to go to the student infirmary first whenever possible, and then to a physician participating in the First Health and preferred provider network (see listing of preferred providers under "First Health".).

Q. Where do I get a claim form?
A. You can print one from this web site or one can be mailed to you on first notice of a claim.

Q. How do I file a claim?
A. Attach your bills to a completed claim form and mail to Bollinger Inc., P.O. Box 727, Short Hills, New Jersey 07078 - 0727. Only one claim form is needed per accident or illness. Subsequent itemized bills may be sent without completing another claim form. Just indicate that you are a Lynn University student and the date of your accident or illness.

Q. With this insurance must I go to college health center first?
A. You are encouraged to go to the student infirmary first whenever that is possible. However, it is not mandatory in order to be covered by this insurance plan.

Q. What if I am out-of-state and I get sick were injured?
A. This insurance plan will cover you for claims which occur out-of-state. You must follow the same procedures in filing a claim as if it happened here in Florida.

Q. How do I get an ID card?
A. It is attached to the enrollment card or print one out from this website.


Q. What is the policy's maximum benefit?
A. The maximum benefit under the mandatory accident plan is $4000. The maximum benefit under the Optional Basic and Major Medical Plan is $54,000 for accidents, and $50,000 for Sickness.
