Q: What is the effective date:
A: September 1, 2008.

Q: What is the policy expiration date?
A: September 1, 2009.

Q: If I missed the original registration or waived coverage initially and now want the coverage, how do I go about getting it?
A: Please contact the college.

Q: How do I know if I am eligible to take the Student Injury & Sickness Insurance?
A: Massachusetts State Law requires that all full-time and qualifying part-time students enroll in a Student Injury and Sickness Insurance plan unless satisfactory evidence is provided to the school that the student is enrolled in comparable coverage for the full academic year.

Q: Can I get the insurance to cover my spouse and/or children?
A: Yes, please refer to the brochure for details

Q: What does this insurance cover?
A: This insurance covers medical expenses which arise from an accidental Injury or a Sickness which occurs while you are covered under this program.

Q: Does this policy pay all medical bills in full?
A: In most circumstances, it will not. There are exclusions and limitations in the policy, so you will need to refer to the brochure.

Q: Can I go to any doctor?
A: Yes you may. However, you are encouraged to go to the Health Center first whenever possible.

Q: Where do I get a claim form?
A: You can pick up a claim form at the Health Center or you can
print one from this link.

Q: How do I file a claim?
A: Attach your bills to a completed
Claim Form and mail to Bollinger Inc., P.O. Box 727, Short Hills, NJ 07078. Only one claim form is needed per accident or illness. Subsequent itemized bills may be sent in without completing another claim form. Just indicate that you are a MA College of Pharmacy Student and the date of your accident or illness.

Q: With this insurance must I go to the College Health Center first?
A: You are encouraged to go to the Health Center first whenever possible. However, it is not mandatory in order to be covered by this insurance plan.

Q: What if I am out of state when I get sick or injured?
A: This insurance plan will cover you for any illness or Injury which occurs out of state. You must follow the same procedures in filing a claim as if it had happened here in Massachusetts.

Q: How do I get an I.D. card?
A:

Q: What is the policy's maximum benefit?
A: $50,000
