pay all medical bills in full? A: No, there
is an annual deductible, as well as limitations and exclusions
for some services.
Q: Can I go to
any doctor? A: Yes, however you will get the
greatest benefit from the plan, if you use a participating
CHN Provider.
Q: Where do I get
a claim form? A: The Student Wellness Center,
or online.
Q: How do I file
a claim? A: Follow the instructions on the
claim form.
Q: Is there a deductible
on this policy? A: Yes, there is a $250 deductible
per policy year.
Q: With this insurance
must I go to the Student Wellness first? A:
No, however visiting the Student Wellness Center first will
reduce your costs, and insure that you receive proper treatment.
Q: What if I am
out of state when I get sick or injured? A:
Obtain the necessary treatment and submit a claim form and bills.
Q: What is the
policy's maximum benefit? A: $25,000 per injury
or illness.
