Q: If I missed the original registration or waived coverage initially and now want the coverage, how do I go about getting it?
A: Access an Enrollment Card at www.bollingerschools.com/tulsa. Complete it and send it with a check for the correct amount made payable to Bollinger Inc. to Wilcox and Jones at the address indicated on the Enrollment Card.

Q: How do I know if I am eligible to take the Student Accident & Sickness Insurance?
A: All registered students are eligible to participate in the plan. All undergraduate students taking 9 or more credit hours (3 or more credit hours in summer), all graduate students taking 6 or more credit hours (3 or more credit hours in the summer), and thesis or dissertation students taking 2 or more credit hours are eligible to enroll in this insurance plan

Q: Can I get the insurance to cover my spouse and/or children?
A: Yes.

Q: What does this insurance cover?
A: Accidents and Sickness subject to the terms and conditions of the plan.

Q: Does this policy pay all medical bills in full?
A: In most cases it will not.

Q: Can I go to any doctor?
A: Yes

Q: Where do I get a claim form?
A: You can print one from this web site (see "Claim Form").

Q: How do I file a claim?
A: Attach your itemized bills to a completed Claim Form and mail to Bollinger, Inc., P.O. Box 727, Short Hills, NJ 07078-0727. Only one claim form is needed per accident or illness.

Q: With this insurance must I go to the College Health Center first?
A: No, however in a non-emergency situation, you are encouraged to go to the Alexander Health Center first whenever possible. Failure to do so will result in a $250 deductible and benefits will be paid at 75%.

Q: What if I am out of state when I get sick or injured?
A: This insurance plan will cover you for any illness or Injury which occurs out of state. You must follow the same procedures in filing a claim as if it had happened here in Oklahoma

Q: What is the policy's maximum benefit?
A: $100,000.
