Q: What is the policy number?
A: The 2008-2009 policy number is A583E.

Q: What is the effective date?
A: August 15, 2008, or the date Bollinger Inc. receives your premium check, whichever is later.

Q: What is the policy expiration date?
A: August 15, 2009. Even if you leave school or graduate, your coverage will remain in force until that date.

Q: If I missed the original registration or waived coverage initially and now want the coverage, how do I go about getting it?
A: You need to contact the college.

Q: How do I know if I am eligible to take the Student Injury & Sickness Insurance?
A: All students are eligible.

Q: Can I get the insurance to cover my spouse and/or children ?
A: Yes

Q: What does this insurance cover?
A: This insurance covers medical expenses which arise from an accidental Injury or a Sickness which occurs while you are covered under this program.

Q: Does the Policy pay all medical bills in full?
A: In most circumstances, it will not. There are exclusions and limitations in the policy, so you will need to refer to the brochure (See sample brochure under "Brochure").

Q: Can I go to any doctor?
A: Yes you can. Discounts on fees for medical services are available through our Preferred Provider Network, First Health ...visit the web site for a directory of providers.

Q: Where do I get a claim form?
A: You can pick up a claim form at the Student Health Service or you can print one from this link.

Q: How do I file a claim?
A: Attach your bills to a completed Claim Form and mail to Bollinger Inc., P.O. Box 727, Short Hills, NJ 07078-0727. Only one claim form is needed per Injury or illness. Subsequent itemized bills may be sent in without completing another claim form. Just indicate that you are an Elizabethtown College Student and the date of your Injury or illness,

Q: Is there a deductible on the Policy?
A: Yes, there is a $50 deductible. This deductible is waived for treatments that are rendered at the Student Health Service or referred by the Student Health Service.

Q: With this insurance must I go to the Student Health Service?
A: You are encouraged to go to the Student Health Service, first whenever that is possible. However, it is not mandatory in order to be covered by this insurance plan.

Q: What if I am out of state when I get sick or injured?
A: This insurance plan will cover you for any illness or Injury which occurs out of state. You must follow the same procedures in filing a claim as if it had happened he in PA.

Q: How do I get an I.D. card
A: I.D. cards are incorporated into the brochure.

Q: What is the policy's maximum benefit?
A: The maximum benefit is $15,000 for Domestic and $25,000 International students.

Q: What does the policy pay for doctors visits?
A: For Sickness the policy covers the expense actually incurred is allowed not to exceed $3,000 for any Sickness. For accidents the policy covers you beginning with the first visit on a Usual & Customary fee basis, not to exceed $3,000 for any one Injury.
