A: June 1, 2008 for Master of Arts and Teaching students and August 1, 2008
for all other students.

Q: How do I know if I am eligible to take the Student Accident & Sickness
Insurance?
A: If you are taking 12 or more credit hours you are eligible for the plan.
Only 6 credit hours needed for Super Seniors.
Q: Can I get the insurance to cover my spouse and/or children?
A:No
Q: What does this insurance cover?
A: Injury and Sickness subject to the terms and conditions of the plan.
Q: Does this policy pay all medical bills in full?
A: In most cases it will not.
Q: Can I go to any doctor?
A: Yes. But discounts are available by using a First Health provider. You can
find a First Health provider on the web at

Q: Where do I get a claim form?

Q: How do I file a claim?
A: Complete the claim form and send it to Bollinger. Send bills and
Explanation of Benefits (EOB)
from other carriers as soon as possible.
Q: Does this policy cover me for my annual physicals?
A: No
Q: Is there a deductible on this policy?
A: No
Q: With this insurance must I go to the University Health Center first?
A: No
Q: How do I get an I.D. card?
A: A claim form can be cut out from the brochure or you can print one on the
website.
Q: What is the policy’s maximum benefit?
A: $1,000 Basic Medical Expense and $3,500 Major Medical Expense.
Q: What do you pay for doctor’s visits?
A: Sickness: $30 for first visit and $15 thereafter (4 visit maximum for
Outpatient).
