A: Injury sustained during the play or practice of intercollegiate athletics.

A: In most cases it will not.

A: Yes. But discounts are available by using a First Health provider. You can

Q: How do I file a claim?
A: Complete the claim form and send it to Bollinger. Send bills and Explanation
Of Benefits (EOB) from
other carriers as soon as possible.
Q: Does this policy cover me for my annual physicals?
A: No
Q: Is there a deductible on this policy?
A: There is a $250 deductible per injury. This deductible will be waived if
there is coverage under the Student Health Insurance Policy …Policy # CIN304E
