Q: What is the effective date:
A:  The effective date is August 27, 2007.

Q: What is the policy expiration date?
A: The expiration date is August 25, 2008
 

Q: How do I know if I am eligible to take the Student Accident & Sickness Insurance?
A: All international students (F-1 Visa) attending Alamo Community College District are automatically enrolled in this accident and Sickness insurance plan.
 

Q: Can I get the insurance to cover my spouse and/or children?
A: Yes, you may.  Please see “Plan 3 Enrollment Card” to enroll on-line.

Q: What does this insurance cover?
A: This insurance covers medical expenses which arise from an accidental Injury or Sickness which occurs while you are covered under this insurance program.

Q: Does this policy pay all medical bills in full?
A: In most circumstances, it will not.  There are exclusions and limitations in the policy, so you will need to refer to the brochure (See sample brochure under “Plan 3 Brochure”).

Q: Can I go to any doctor?
A: Yes, you may.  However, you are encouraged to go to a physician participating in the
First Health Preferred Provider Network (See listing of preferred providers under “First Health”).

Q: Where do I get a claim form?
A: You can  print one from this web site (see "Claim Form") or one will be sent to you upon our receipt of notice of your claim.

Q: How do I file a claim?
A: Attach your itemized bills to a completed Claim Form and mail to Bollinger, Inc., P.O. Box 727, Short Hills, NJ 07078-0727. Only one Claim Form is needed per accident or illness.

Q: With this insurance must I go to the College Health Center first?
A:  No, you need not. However, you are encouraged to go to a physician participating in the
First Health Preferred Provider Network (See listing of preferred providers under “First Health”).

Q: What if I am out of state when I get sick or injured?
A: This insurance plan will cover you for any illness or Injury which occurs out of state. You must follow the same procedures in filing a claim as if it had happened here in Texas.

Q: How do I get an I.D. card?
A: It is attached to the Brochure. You can cut it out and carry it with you.

Q: What is the policy's maximum benefit?
A: $25,000.
